Hospital Pricing and Surgeon Evaluation Information
April 1, 2006 – March 31, 2007

Product Purchase Information:

  • The LIBRA™ Dynamic Knee Balancer is currently available in the following sets supporting the listed knee systems:
    1. LIBRA™ BV Set (Biomet® Vanguard™ Knee System) Cat.# 60-0610
    2. LIBRA™ DS Set (DePuy® pfc-Sigma™ Knee System) Cat.# 60-1210 This set supports both fixed and rotating platform tibial implants
    3. LIBRA™ SD Set (Stryker® Duracon® Knee System) Cat.# 60-3710
    4. LIBRA™ ST Set (Stryker® Triathlon™ Knee System) Cat.# 60-3720
    5. LIBRA™ ZNG Set (Zimmer® NexGen® Knee System) Cat.# 60-4810
    6. LIBRA™ 3D Set (Encore® 3DKnee™) Cat.# 60-1520
  • Hospital list price for a LIBRA™ set is $7,950 with payment terms net 60 days.
  • Contact the Global Customer Service Center at Synvasive to place an order or request additional information on a specific LIBRA™ set. Domestic Surgeon Evaluation Program:

  • A 30 day evaluation program is available to interested surgeons and their affiliated hospitals for the fee of $500 per set.

How to Order the Evaluation Sets:

  • Complete and forward the evaluation request form to Synvasive. Be sure to identify the LIBRA™ set required.
  • The evaluation set will be shipped UPS 2-Day Air upon receipt of a purchase order for the $500 evaluation fee.

Options following the evaluation:

Purchase:
The $500 / set evauation fee will be applied toward the purchase price of $7,950. An invoice for $7,450, or the remaining balance will be forwarded to the hospital with 60 day terms.

Rental Option:
The hospital has the option to convert the purchase balance due into a lease agreement at their choosing. The monthly rental fee in this agreement is $375 per set and requires entering into a lease agreement with Synvasive, contact customer service for details.

No Risk Return:
If after the 30 day evaluation the LIBRA™ set does not provide the anticipated assistance in surgery as expected, simply contact Syvasive’s customer service department at 800-925-2337 and we will arrange to pick-up the set (sterile cleaning required prior to pick-up).
